San José and Santa Clara County: where to find meals and in-home support for older adults

The city meal program, Sourcewise, and county IHSS offer different entry points for nutrition, mobility, and daily in-home care.

San José and Santa Clara County offer support for different situations: lunch at a community center, delivered food for someone who cannot cook, and Medi-Cal-authorized in-home hours. Before calling, identify whether the person can leave home and whether the need is food, companionship, or help with daily activities; each route has a different assessment.

For a weekday meal, the city runs a Senior Nutrition Program at 14 sites. The city program is open to adults age 60 and older, requires a yellow nutrition card, and may require reservations depending on the site. The May 2026 city menu suggests a five-dollar contribution for eligible participants and a fifteen-dollar fee for guests under 60; check the current menu because rates and availability can change.

If leaving home or preparing food is no longer possible, Sourcewise coordinates Meals on Wheels for county residents. Its page requires age 60 or older, Santa Clara County residency, homebound status, and difficulty shopping or cooking without help. Intake begins by phone; there is no required fee, although a daily contribution is suggested. Weekly deliveries arrive frozen, and the program cannot tailor meals to every dietary restriction.

When the need goes beyond food, the county administers In-Home Supportive Services. IHSS may authorize tasks and hours for people who meet Medi-Cal rules and need assistance to live safely at home. A county social worker visits, determines the tasks and time, and sends an approval letter. The recipient can then hire someone they know or use the Public Authority registry.

California also funds community services through its Area Agency on Aging network. Sourcewise serves as a regional agency and can guide residents to caregiver support, benefits, and community connections; IHSS, by contrast, is a formal county decision within Medi-Cal. Medicare generally does not pay for ongoing nonmedical daily care. The California Department of Aging describes community programs, and the federal Eldercare Locator can find other agencies.

Start with the city center for lunch, Sourcewise for delivery and referrals, or the county when a personal-care assessment is needed. Have the person’s age, address, Medi-Cal status, ability to cook, mobility, and required tasks ready. Ask who determines eligibility, whether a contribution is suggested, how long the process may take, and how to request review if a decision does not reflect the person’s needs.

Bottom line

In San José, the first choice depends on whether the person can reach a meal site, needs food delivered, or needs county-authorized personal assistance at home.
