About Sourcewise Community Resource Solutions

Sourcewise is a nonprofit and a regional resource for older adults and caregivers in Santa Clara County. Its Meals on Wheels program delivers frozen food to homebound residents, while other services support Medicare decisions, caregivers, transportation, senior employment, and connections to community help. The same organization can guide a family comparing more than one option.

Meal delivery generally requires the person to be at least 60, live in Santa Clara County, live alone or with a spouse who is also homebound, and need help or supervision with multiple daily activities. A nutrition coordinator conducts a phone assessment. Weekly deliveries include five meals and accompanying items; the food arrives frozen, and Sourcewise may loan a microwave if the participant does not have one.

There is no required fee. Sourcewise suggests a three-dollar daily contribution but says meals will not be denied to someone who cannot pay. Routes depend on the address, the participant must be available to receive the delivery, and drivers cannot leave food outside, so absences need advance planning. The organization cannot accommodate individual dietary preferences or restrictions, but participants can review ingredients and sodium and sugar information with a doctor before enrolling.

To begin, call (408) 350-3200, option 1, to speak with a Community Resource Specialist about meals and other help. Sourcewise also posts a Meals on Wheels interest form. If someone does not qualify, the organization says it can connect them to other county resources rather than ending the conversation.

Sourcewise also serves as an Area Agency on Aging, offering guidance on Medicare counseling, Medi-Cal, caregiver support, and community connections. That guidance can reduce scattered calls, but it does not replace a formal decision by a state or county agency. Ask who delivers each service, what geographic boundary applies, and whether a waitlist or suggested contribution is involved.
