About Minnesota Aging Pathways — Minneapolis and Hennepin County

Minnesota Aging Pathways—formerly the Senior LinkAge Line—is a free service of the Minnesota Board on Aging, working with regional agencies. It serves older Minnesotans statewide, including families in Minneapolis and Hennepin County. Call 800-333-2433. The Board on Aging’s state office is listed at 540 Cedar Street in St. Paul, but guidance starts by phone or through the referral portal, not by travelling to that address.

Specialists provide guidance on Medicare, prescription costs, Medical Assistance, long-term-care planning, housing options, and community services. They can help compare choices and connect people with meals, transportation, home modifications, caregiver support, or financial and legal assistance. Under Resource Coordination, an individual, family member, or provider can submit a referral; a coordinator discusses needs, helps build a practical plan, and follows up. Eligibility depends on the specific pathway and assessment.

A call can help organise next steps: what coverage is in place, who assesses the need, and whether support is state-run, county-administered, or provided by a local organisation. Aging Pathways also carries statewide SHIP and ADRC designations, among other roles. It does not itself deliver every service or guarantee funding. Have the ZIP code, coverage cards, and a list of tasks needing help ready; ask whether to submit an online referral or contact Hennepin County directly.
