Lake Arlington Family Medicine is a family medicine clinic that has been part of the Arlington, Texas community since 2000. Medical Team

Dennis Poquiz, MD, FAAFP, serves as medical director and supervises the team of providers. He is board certified by the American Board of Family Medicine and is a Fellow of the American Academy of Family Physicians; he has been in private practice in Arlington since 1999.

The team also includes four nurse practitioners (NP-C):

  • Kristina Salcedo, NP-C: originally from San Diego, California, she moved to Texas in 2006 and graduated with honors in December 2017 from the University of Tyler. She has over 22 years of previous critical care experience and focuses her interest on disease prevention and improving patients' quality of life.
  • Janelle Rosenberger, NP-C: earned her BSN from Texas Tech University and, after six years of hospital experience, completed her Master of Science in Family Nurse Practitioner in 2019 at The University of Texas at Arlington. Her practice is oriented toward patient-centered care, with an emphasis on prevention and wellness.
  • Debra "Chris" Ray, NP-C: earned a Master of Science, Family Nurse Practitioner in 2014 and a Bachelor of Science, Nursing in 2005, both from Texas Woman's University (Dallas). She has practiced at Lake Arlington Family Medicine since 2015 and works from detailed health histories, physical exams, and a close patient relationship.
  • Daniel Purifoy, NP-C: from the Houston area, he graduated in Biology from Texas A&M in 2017, earned his BSN from the University of Texas at Tyler in 2019, and completed his Master of Science in Nursing at that same university in 2024. He previously worked as an emergency medical technician and as an emergency and critical care nurse at JPS Hospital.

Patient Documentation

The clinic provides forms and documents useful to patients, such as new patient paperwork, permission to treat a minor, medical records release, a blood pressure log, the HIPAA form, and the notice of privacy practices, also available in Spanish as "Notificación de Prácticas de Privacidad".
